Residents at The First Apartments benefit from having a Service Coordinator as an available resource.  The Service Coordinator’s role is to coordinate and link residents to needed services that will assist them to age in place.  Scheduled speakers from the network of community agencies provide information about their services.  Appropriate referrals for service are facilitated with residents and their families as part of case management.  The Service Coordinator advocates for residents with community agencies, insurance companies, Medicare, Social Security, etc. to problem solve and bring about resolution.  Educational presentations, annual flu and pneumonia inoculation clinics, assistance with Medicare Part D annual enrollment are available to residents through the Service Coordinator.
